What to expect from your first visit at SJ/C Physician Network - Metro Surgical
Welcome to St. Joseph's/Candler Physician Network - Metro Surgical. Surgery is different for everyone but your surgeon will explain the procedure to you. We encourage you to ask any questions you have about your upcoming surgery.
When you come in for your first appointment, we also ask that you please bring the following important information:
- Relevant medical records from other medical providers who have treated you for your current condition.
- Any relevant X-rays, scans and/or test results.
- A list of all your current medications.
- All medical insurance cards.
- An insurance authorization or a written referral from your primary care doctor, if required by your insurance company.
Please come to your appointment 15 minutes early to fill out any necessary paperwork prior to your appointment.
