At Georgia Ear, Nose & Throat Specialists, your first impression of us starts with your first visit. We make every effort to ensure your appointment goes as smoothly and efficiently as possible and to help with that, we have provided the following information containing important steps to take before and on the day of your appointment. We encourage you to call us with any questions you may have and look forward to seeing you soon.

Important information for your appointment

To give you a smooth and stress-free experience on the day of your appointment, we’ve outlined some essential information to help you prepare. Following these guidelines will assist in making your visit as seamless as possible, allowing us to focus on providing you with the highest quality of care. Review the details below carefully so you can arrive ready and informed.

  • Patient Packet: Download and complete your patient packet here.
  • Arrival Time: Please arrive 15 minutes before your appointment to ensure a smooth registration. If you arrive more than 15 minutes late, please call our office to reschedule or confirm if the physician or audiologist can still see you.
  • Required Forms and Documents: Bring your completed patient forms, a map for reference, and all necessary documents, such as your insurance card and any medical records (CT scans, MRIs, X-rays, Sleep Studies). Not having these items may result in extended waiting times.

Insurance and payment

  • Co-Payments & Deductibles: Be prepared to pay any required co-pays or deductibles during service. Ensure you are familiar with your insurance requirements to avoid any surprises.
  • Referral Requirement: If your insurance requires a referral, please request it from your primary care physician before your appointment. Without it, we may need to reschedule your appointment.
  • Insurance Card: Remember to bring your insurance card; without it, we must reschedule.

Additional policies

  • Patient Portal: After your first visit, you can create an account on our patient portal. This is an excellent source for checking test results, viewing upcoming appointments and communicating with your provider. Create your account now
  • Appointment Cancellations: If you cannot attend, please cancel at least 24 hours in advance. A $25 fee will apply for no-shows.
